It was supossed to be Doki Doki Panic but Nintendo didn't think it would sell so they made it into Super Mario World 2. They changed the name to Super Mario:the LOST levels. i think that it is now Super Mario Advance ( the first one)

Super mario advance is a followup to Super Mario brothers 2(USA), or in Japan, Super Mario Brothers USA. Super Mario Advance was a completely new game made years later.
Doki Doki panic was only released in Japan, yes, but I have no idea why it wasn't released in america. The REAL Sumer Mario Brothers 2 (Lost levels) was considered to hard for the Americans, so it was scrapped, and all they did was change the sprites in Doki Doki Panic, and bam, super Mario Bros 2(USA)

I'm pretty sure it was because the Japanese Super Mario Bros. 2 was considered too similar to Super Mario Bros., and they didn't think it would sell. Our feeble American brains couldn't possibly fathom a sequel looking too much like the original, let alone picking up where the first left off :P
